(A)  FOR OPEN MERIT SEATS
(i) Three (03) attested copies of Matriculation (SSC) Certificate or result card issued by concerned BISE/Equivalence Certificate of O-Level, etc., issued by IBCC.  (ii) Three (03) attested copies of HSSC Pre-Medical result card issued by concerned BISE /Equivalence Certificate of A-Level/12th Grade,etc. issued by IBCC. (iii) Three(03) attested copies of transcripts of A-level/12th Grade, etc., (in case of foreign qualifications) issued by concerned foreign university/high school/board. (iv) Three (03) attested copies of result card of relevant Provincial Admission Test conducted by concerned Provincial Admission Authority in 2018. (v) Three (03) attested copies of Domicile Certificate of the candidate. (vi) Three (03) attested copies of CNIC/B-Form of the candidate.  (vii) Three (03) attested copies of CNIC of Father / Guardian.  (viii) Four (04) recent color photographs of the candidate, Size: 3.5cm wide & 4.5cm high (One to be pasted on the form and then attested on the front; other three attested on the back and attached with the form).  (ix) Original Undertaking on Rs.100/- stamp paper (as per template given as Annexure P-1 on UHS website:  www.uhs.edu.pk  (x) Three (03) attested copies of Hifz-e-Quran Certificate issued by a registered Madrassa (only for candidates claiming to be Huffaz-e-Quran).

(B)  FOR FOREIGN/SELF-FINANCE SEATS
(i)  Three (03) attested copies of Matriculation (SSC) / Equivalent Certificate issued by IBCC;  (ii) Three (03) attested copies of HSSC Pre-Medical/12th Grade or Equivalent Certificate issued by IBCC. The candidate must have studied and passed HSSC/12th Grade Examination or Equivalent from outside Pakistan and has stayed in the foreign country for the whole duration of the said course; (iii) Three(03) attested copies of transcripts of HSSC/12th Grade/Equivalent issued by concerned foreign Institute; (iv) Three (03) attested copies of result card of relevant Provincial Admission Test conducted by concerned Provincial Admission Authority in 2018/SAT II/MCAT ® International scores, as the case may be. SAT II (should not be less than 550/800 in each of Biology, Chemistry and Physics/Mathematics)/ MCAT ® International (should not be less than 500/528) scores declared before 1st November, 2016 and after 1st November, 2018, shall not be acceptable for this year's admissions; (v) Three (03) attested copies of current valid Pakistani passport OR valid foreign passport, as the case may be, of the candidate; (vi) Three (03) attested copies of valid CNIC/Passport/ NIC/ NICOP/ POC of Father / Guardian;  (vii) Four (04) recent color photographs of the candidate, Size: 3.5cm wide & 4.5cm high (One to be pasted on the form and then attested on the front; other three attested on the back and attached with the form);  (viii) Original Undertaking on Rs.100/- stamp paper (as per template given as Annexure P-1 on UHS website:  www.uhs.edu.pk  (ix) Three (03) attested copies of Hifz-e-Quran Certificate issued by a registered Madrassa (only for candidates claiming to be Huffaz-e-Quran).
IMPORTANT

1. Please note that the photocopies of the document attached with the Application Form must be attested by a Government Officer in BS-17 or above. The stamp of the officer must bear his/her full designation and current place of duty.  2. All the documents mentioned above are “MANDATORY” in nature. If any of the document is found missing with the Application Form, it will be considered as “Incomplete” and shall be rejected. 3. Applicants must ensure that:   The photocopies of documents are taken on A4 size paper only.  The print is clear and legible.  Both sides are photocopied and attested if the original document is printed on both sides. 4. The photocopy of any document required in “Original” shall not be acceptable.
